Case Study

68-year-old male admitted to Riverton Rehabilitation and Healthcare Center from Lehigh Valley Muhlenberg Hospital with admitting diagnosis of Hypoxia, Encephalopathy, and weakness. He has a history of a Left BKA, stroke and heart failure. Patient admitted to hospital requiring Thoracentesis and High Flow Oxygen therapy.

Nursing Interventions

Monitor Fluid Balance – 1800ml fluid Restriction, Daily Weight Monitoring
Medication Management – Insulin, Apixaban, Coreg, Lasix, Eliquis, Carvedilol
Maintain Safety – History of stroke w Left BKA Amputation
Monitor Labs – BBG, HGB, CBC, BMP

Respiratory Interventions

Maintain Adequate Oxygenation – Wean as Tolerated – GOAL MET
Pacing and Endurance with Exertion

Rehabbing Care Interventions

Patients short term rehab stay was management by Riverton’s in house Physiatrist Dr. Demetrice Ward. Patient displayed ability to don and doff his prosthetic. He is successfully ambulating 150 ft x 2 with a front wheeled walker. Patients weight remained stable, and he gained the strength needed to return home with his wife.

Physiatrist in the community is Dr. Stanley Yoo, LVPG Physiatry. He will continue to follow with PCP, Dr. Brian Shamblin, Pulmonologist, Dr. Brian Burlew in the community and LVHN Home Health Services.
